LIWSA Online Youth-Senior Player Permit Form

  • Coaches and players must verbally agree before using this form.
  • All submissions are subject to final approvals from the junior team's governing body and LIWSA
  • A Youth Permit allows a youth registered (BCSA) player to play on a game permit, for an adult team. Each individual player may move up for an unlimited number of games within a playing season. A maximum of three youth permits may be used in any one game. Please familiarize yourself with the Rules and Regulations (Rule 10) regarding the use of the Youth Permit. Disciplinary action may be taken against those who fail to observe the regulations.
  • All Youth Permits must be completed and submitted to the junior teams governing body 24 hours before the game. LISA's office hours available for submitting Youth Permits are Monday - Friday 10:00am to 4:30pm. Completed and approved permits once returned to requesting team official must be printed and presented in duplicate to the game official.
